For other diseases, replace with a Minute of the Medical Appraisal Council appraising the degree of labor capacity impairment at 81% or higher.
  • If there are fees for medical appraisal, provide the invoice and receipt for the medical appraisal fee along with a list of the contents of the medical appraisal (original) from the medical appraisal facility.
  • Original Personal Declaration regarding the period and location of service in the military with regional allowances for those who served in the military before January 1, 2007, in areas with regional allowances, where the social insurance code does not fully reflect the information for calculating regional allowances.
