  • Application form for an extract of household registration
  • Original copy of one of the following documents: Passport, National ID Card, Citizen ID Card, or other valid personal identification documents issued by authorized agencies.

Note: Submit your application to the same place where your birth was initially registered.

Upon receiving your application for an extract of household registration, the agency storing the original Household Registration Book will issue you an extract of your birth certificate.

  • The extract contains the same information as the original Household Registration Book.
  • The extract is legally valid and can be used in place of the original birth certificate for various transactions.

Procedure for re-registering birth

According to Decree 123/2015/ND-CP, Article 26, re-registration of birth can only be done under the following circumstances:

  • The birth was registered before January 1st, 2016;
  • Both the Household Registration Book and the original birth certificate are lost.
  • Once the above requirements are met, follow these steps to re-register your birth:

Step 1: Prepare the necessary documents

  • Form for re-registering birth: This form must include a commitment statement regarding the re-registration due to the loss of the original document.
  • Copies of relevant records and documents related to the person applying for re-registration.
  • National ID Card, Citizen ID Card, or Passport;
  • Household Registration Book, Temporary Residence Book, or other documents proving residence;
  • Diplomas, certificates, transcripts, or other educational records issued or verified by authorized educational institutions;
  • Documents proving the parent-child relationship.

If the applicant is a military officer, police officer, or civil servant, the following additional documents are required: Confirmation from the unit leader regarding

  • Full name;
  • Gender;
  • Date of birth;
  • Ethnicity;
  • Nationality;
  • Place of birth;
  • Parent-child relationship.

Step 2: Submit the application to

  • The People’s Committee (PC) of the commune/ward/township where the birth was initially registered
  • PC of the commune/ward/township where the applicant is a permanent resident.

Step 3: Review by the authorities

Case 1: The applicant submits the application to the same place where the birth was initially registered

Within 5 working days from the date of receiving the application, the justice-household registration official will review and verify the application. If the re-registration request is valid, a new birth certificate with the “re-registered” seal will be issued.

Case 2: The applicant submits the application to a different PC from the initial registration location.

The justice-household registration official will report to the PC Chairman, who will then send a written request to the PC where the birth was initially registered, asking them to verify the retention of the Household Registration Book.

Within 5 working days from receiving the request, the PC of the initial registration location will conduct a verification and provide a written response regarding the retention or non-retention of the Household Registration Book.

Within 3 working days from receiving the verification result that the Household Registration Book is not retained, the justice-household registration official will proceed with the birth re-registration if the application is complete.

Note: The content of the re-registration will be determined based on the documents mentioned in Step 1.
