- 01 notarized copy of ID card on both sides or notarized copy of citizen identification card on one side of an A4 paper
- 02 exam registration forms numbered 1 and 2
- 04 ID photos (4×6) with full name and date of birth written on the back, enclosed in an envelope
- 02 envelopes with affixed stamps and clearly labeled with the candidate’s name, address, and phone number
- Certificates of priority and encouragement (certificates of provincial excellent students, national excellent students, certificates of students from special difficult areas with notarized copies of household registration book, etc.)

- In the case of independent candidates who have not graduated from high school:
- 02 exam registration forms numbered 1 and 2

Section GDÐT…CODE: Candidates should write the name of the Department of Education and Training in the corresponding blank space for the examination location belonging to the Department of Education and Training, and then fill in the 2 digits of the Department’s code in the following 2 empty boxes. The Department’s code is determined by the Ministry of Education and Training.
Ballot number section: Candidates do not need to fill in this section.
Section 1 and section 2: Fill in the required information as instructed on the National High School Graduation Exam registration form.
Section 3:
- Place of birth: Clearly write the name of the province or city. If born outside of Vietnam, write the name of the country in Vietnamese.
- Ethnicity: Accurately write the ethnicity according to the birth certificate.
- Nationality: Mark (X) in the blank box if holding foreign citizenship.
Section 4: If using the new model of Citizen Identification Card/Identity Card, fill in all 12 digits in the corresponding boxes. For the old model, fill in the last 9 boxes and leave the first 3 boxes blank.
Section 5: Look up the registration location to find the correct city code, district code, and commune/ward code for the candidate’s current residential address, and fill them in accordingly.
Section 6: Write the name and address of the school, district, province/city, and fill in the province code of the school in the first 2 boxes and the school’s code in the following 3 boxes.
If the candidate has studied abroad, write the corresponding province/city code for the province/city currently residing in Vietnam, and use the code 800 for the high school.
Class name section: Clearly write the current class, such as 12A5. If it is an independent candidate, write “TDO”.
Section 7: Declare the phone number and email accurately.
Section 8: Clearly write the contact information, including the name, address, and phone number of the contact person.
Section 9: Candidates who want to obtain the exam results for university admission or pedagogical college admission should mark (X) in the adjacent box.
Section 10: Mark (X) in the corresponding box according to the current program the candidate is studying – high school or Continuing Education program high school level.
Section 11: For independent candidates, mark (X) in one of the two boxes to determine whether they have graduated from high school or not (as of the registration date).
Section 12: Write the name of the Examination Council and the code of the Examination Council determined by the Ministry of Education and Training according to the corresponding Examination Council registered by the candidate.
Section 13: Candidates who are currently studying in 12th grade at a certain school should submit their application at that school. Others must send their application to the location designated by the Department of Education. The registration unit code should follow the instructions of the registration place for the exam application.
Section 14: Mark (X) in the corresponding box for the chosen exam subject. For the Foreign Language exam, candidates should fill in the code for the selected language in the corresponding blank space. The language codes are as follows: N1 – English, N2 – Russian, N3 – French, N4 – Chinese, N5 – German, N6 – Japanese, N7 – Korean.
Section 15: If you want to be exempted from the Foreign Language exam, candidates need to write the type of foreign language certificate that meets the requirements or specify that they are members of the national team participating in the International Foreign Language Olympic.
Section 16: Candidates who have taken the exam in the previous year and want to reserve scores for certain subjects should mark (X) in the corresponding box.
