Article 27 of Official Dispatch No. 2089/VBHN-BHXH states that the reissue of social insurance books only applies in cases of loss or damage, consolidation or change of book number, change of name or date of birth, or for those who have received social insurance benefits once but still have unemployment insurance contributions remaining.
Although a reissue of the social insurance book is not necessary, individuals are advised to complete the Declaration Form (TK1 – TS) to adjust and supplement the identification number for data consistency in the system.
This form can be submitted to the individual’s place of work or directly to the social insurance agency if they have left their job but wish to retain their social insurance contributions.
